Are you responsible for an older person's care but don't know where to begin?
The Care Managers at Independent Living Solutions offer consultation, coaching, research, and education for caregivers of older adults who are in need of services because of physical or cognitive changes.
We can help identify the priorities, outline the options available, and develop an action plan with the steps necessary to manage
-a recent diagnosis or progression of a medical or neurological disease
- a series of worrisome events such as frequent falls
- a noted decrease in memory and/or ability to manage home or personal care
- the negative effects of increased isolation
- refusals to accept much needed assistance
As each family and situation is different, the care management approach is individually tailored to meet the goals of maintaining as much self-determination and dignity as possible for the older adult. Consideration is always given to the goals of the older adult while taking into consideration their safety, informal supports, and financial resources to create a plan that meets their needs for the present while looking ahead to make sure the plan is sustainable. Consultations can be a one-time conversation to identify where and how to begin or ongoing sessions to assist in decision making, problem solving, or addressing new issues as they arise. It's up to you.
Independent Living Solutions, Inc was founded in 1999 and is owned by Eileen Berman. Eileen is a licensed Occupational Therapist with a Masters degree from Boston University, and certification as a Geriatric Care Manager. She has 25 years of successful care management experience working with clients and their families on the North Shore in MA. She has been a member of the National Association of Aging Life Care Professionals since 2000.
